Shoulder Pain: When Is It Your Shoulder… And When Is It Actually Your Neck?

Many Cases of Shoulder Pain Start Somewhere Else Than the Shoulder

If you’re dealing with shoulder pain, it’s natural to assume your shoulder is the problem.

However, that’s not always the case.

In fact, many patients who visit my office in Chicago discover that their shoulder pain actually begins in the neck, upper back, or surrounding muscles. As a result, treatments focused only on the shoulder often provide temporary relief but fail to solve the real problem.

You may notice:

  • Pain when reaching overhead

  • Difficulty sleeping on one side

  • Pain when lifting objects

  • Stiffness when reaching behind your back

  • Weakness in your arm

  • Pain that travels from your neck into your shoulder

If any of those symptoms sound familiar, keep reading.

In my Chicago office, one of the most common surprises I see is that shoulder pain often starts in the neck or upper back rather than the shoulder itself.

The source of your shoulder pain may not be what you think.

What Causes Shoulder Pain?

Shoulder pain can develop for several different reasons.

While some cases involve injuries to the shoulder itself, others originate elsewhere in the body.

Some of the most common causes include:

Rotator Cuff Irritation

The rotator cuff is a group of muscles and tendons that help stabilize your shoulder.

Over time, repetitive movements, poor posture, sports activities, and age-related changes can irritate these tissues.

According to the Mayo Clinic, rotator cuff injuries are one of the most common causes of shoulder pain and can range from mild irritation to significant tendon damage.

Common symptoms include:

  • Pain reaching overhead

  • Difficulty putting on a jacket

  • Pain when sleeping on your side

  • Weakness lifting objects

Frozen Shoulder

Frozen shoulder develops when the shoulder joint becomes stiff and loses mobility.

Patients often describe:

  • Significant loss of motion

  • Difficulty reaching overhead

  • Trouble reaching behind their back

  • Gradually worsening pain

Frozen shoulder frequently develops after injuries, surgeries, or periods of reduced movement.

Shoulder Impingement

Sometimes the tissues inside the shoulder become compressed during movement.

This condition often causes:

  • Sharp pain when lifting your arm

  • Pain during exercise

  • Clicking or catching sensations

  • Pain near the front or side of the shoulder

Arthritis

As joints age, normal wear and tear can affect the shoulder.

Arthritis-related shoulder pain may cause:

  • Aching discomfort

  • Stiffness

  • Reduced range of motion

  • Pain that gradually worsens over time

When Shoulder Pain Actually Comes From the Neck

This is one of the most overlooked causes of shoulder pain.

Nerves exit the neck and travel into the shoulder, arm, and hand. Therefore, irritation in the cervical spine can create symptoms that feel exactly like a shoulder injury.

Patients are often surprised when a neck problem creates:

  • Shoulder pain

  • Arm pain

  • Numbness

  • Tingling

  • Weakness

  • Reduced grip strength

As a result, treating only the shoulder may not provide lasting improvement.

The Role of Posture

Modern lifestyles place tremendous stress on the neck and shoulders.

Many people spend hours every day:

  • Looking down at phones

  • Working on computers

  • Driving

  • Sitting at desks

Over time, these habits create forward head posture and rounded shoulders.

Consequently, the muscles surrounding the neck and shoulders must work harder throughout the day.

This added strain frequently contributes to chronic shoulder pain.

A Common Patient Example

A patient recently visited my office after several months of shoulder pain that began during weight training.

He assumed he had injured his rotator cuff. However, after evaluating his posture, neck movement, shoulder mechanics, and muscle function, we discovered the primary problem was coming from his neck and upper back.

Once we addressed those underlying issues, his shoulder pain gradually improved.

This is exactly why a proper evaluation matters.

Signs You Should Have Shoulder Pain Evaluated

Consider scheduling an evaluation if you experience:

  • Pain lasting longer than two weeks

  • Difficulty lifting your arm

  • Weakness

  • Night pain

  • Numbness or tingling

  • Pain traveling into the arm

  • Limited shoulder movement

  • Pain that continues to return

The sooner you identify the source of the problem, the easier it becomes to create an effective treatment plan.

What I Look For During an Evaluation

When evaluating shoulder pain, I don’t focus only on the shoulder itself.

Instead, I examine:

  • Shoulder mobility

  • Rotator cuff function

  • Posture

  • Neck movement

  • Upper back mobility

  • Muscle imbalances

  • Movement patterns

  • Nerve involvement

This comprehensive approach often helps identify contributing factors that might otherwise be missed.

If you’ve never been to a chiropractor before, here’s what to expect during your first visit.

What Can Help Shoulder Pain?

Treatment recommendations vary depending on the cause.

Depending on your findings, recommendations may include:

  • Improving posture

  • Mobility exercises

  • Stretching tight muscles

  • Strengthening weak muscles

  • Activity modifications

  • Chiropractic care

  • Movement retraining

The goal is not simply to reduce symptoms.

Instead, the goal is to address the underlying cause whenever possible.

When to Schedule an Evaluation

If shoulder pain is affecting your workouts, sleep, work, or daily activities, it may be time to find out what’s really causing it.

Many patients spend months trying stretches, exercises, or rest without fully understanding why the problem keeps returning.

A proper evaluation can help identify the source and determine the most appropriate next steps.

If you’re searching for care in Chicago, learning how to choose the right chiropractor can help you make a confident decision.

Final Thought

Shoulder pain is often treated as a shoulder problem.

However, many cases actually begin in the neck, upper back, posture, or movement habits.

Before assuming something is torn or permanently damaged, take the time to identify the true source of the problem.

The right evaluation can save months of frustration and help you make more informed decisions about your health.
